My fanon of your fanon:
She only writes contracts and only with all parties present and paying at least part of her fee. That way she never sees the inside of a courtroom and is legally obligated to look out for everypony’s interests. She strives to write the fairest contracts she can and make sure everyone understands everything very clearly.
For those too poor to pay a mostly-token hundred bits, and too proud to only pay a completely token one bit, she does keep a stock of syringes and has training to use them safely and with as little pain as possible. While she lacks any great training, let alone certification, she will, with careful disclaimers direct anyone she tastes malnutrition, anemia, etc on to seek out a check-up, and has a small fund from Rarity to pay for those reluctant to incur such a cost. Funds for actual treatment can be obtained with a phone-call to Rarity (via her secretary who will go get her) for immediate payment. Rarity DOES like to use her money well, and, even over the phone has a better ability than Fluttershy to detect leeches who could actually pay for the treatment themselves.
Oh… and woe betide any who break one of Fluttershy’s contracts to try to take advantage of anyone… the mesmerization clause exists in all her contracts since after the first time she used The Stare (one month in a minimum security prison and a two thousand bit fine), and she is too shy to explain that it works in combination with intimidation rather than the more subtle mind-snaring of her kin of the night.
